Mortgage Loan Application Checklist
Be sure to review the checklist before meeting with the loan officer 
Mortgage Application Checklist
1. Residence History
You will be asked to provide the street address for every residence you have lived in during the past two years.
If you are currently renting, bring:
Landlord's name
Landlord's address
Landlord's phone number
If you own your current residence, bring:
Mortgage lender's name
Mortgage account number
This information will be used to get the payoff amount for your present mortgage.
2. Employment History
Bring your paycheck stubs with
Employer's name
Employer's address
Employer's phone number
Income Information
Also bring:
Gross monthly salary or other income
Year-to-date pay stub
W2s for the past one year. (Depending on your situation another year may be required.)
Signed tax returns if you worked on commission
Signed tax returns if you wish to use interest, dividend, or bonus income to qualify for your loan
If you're self-employed, you should bring:
Signed tax returns for the past two years
Current Profit & Loss Statement
Current Balance Sheet
If you are not currently employed, bring:
Proof of other sources of income*
4. Deposit, Retirement, and Equities Account Information
For all Checking, Savings, IRA, Retirement, 401(k), Stock, and Mutual Fund accounts, supply
Name and address of the financial institution holding the account
Account number
Account balance
Statements for the past two months
5.Debt Information Debts will be determined from your credit report.
6. Real Estate Information
For all real estate you own, bring:
Address of the property
Market value
Loan information
Copies of leases on any rental property you own
7.Personal Property Information
Estimated value of any personal property such as autos and home furnishings.
8. Sales Contracttbr>
Bring the completed and signed sales contract for the property you are purchasing. Contract should be signed by both seller(s) and buyer(s)
9.Fees
Be prepared to make a payment for:
Property appraisal
Credit report
Flood certification
These fees are determined by the lender and local customs. You should expect to pay $250-$350 for an appraisal, $15 for the credit report and about $8 for the flood certification.
10. Identification
Bring one of the following forms of government-issued photo ID:
Drivers license
State-issued non-drivers ID
US military ID
11. VA Loan Applicants If you're applying for a VA Loan, bring:
Military discharge papers (Form DD214)
Certificate of Eligibility
*Alimony, child support or separate maintenance income need not be revealed if you do not choose to have it considered as income for repaying the loan.
