As owner and the undersigned as Builder, entered into a certain agreement for the construction of a certain building upon the following lands in County, , to-wit:
WHEREAS, said building has been fully completed and said contract fully performed, and final payment under said contract has become due the undersigned from the owner.
NOW, THEREFORE, in compliance with the CONSTRUCTION LIEN LAW, of the State of Florida, and to enable the undersigned to obtain final payment, the undersigned states on oath that all lienors contracting directly with or indirectly employed by the undersigned, on or in connection with said building, and under and in connection with said contract, have been paid in full with the proceeds from this final payment

I, or we say that the persons, firms and corporations who have executed the above waiver and release of liens include all of the persons, firms and corporations, with whom or with which I or we have contracted for services, labor or materials in the construction or repair of improvements on the property described in said Release of Waiver; that as of the date of this affidavit, such work has been fully completed and accepted by me, or us: that the Builder, if any and all parties with whom I, or we, have contracted have been paid in full; that at the time of paying said parties in full, I or we, had no notice of any claim of any sub-contractor, laborer or materialman; and that I or we, do not now have any notice of any claim of any subcontractor, laborer or materialman other than those shown in the Builder's Affidavit above. This affidavit is made for the purpose of inducing Lender to disburse the final proceeds of the construction loan.
